Please provide us with the following information to obtain a password. Once you receive a password by e-mail, use the Member Profile area to modify your personal information and change it to something more familiar. You will use your e-mail address and password to log in to www.cdaa.ca in the future.

CDABC: Provincial ID numbers must contain five numbers and start with a 'M'. For example, if your provincial ID is 123 then you must enter M00123. If your provincial ID is 4567 then you must enter M04567.

CDABC Students: Provincial ID numbers must start with S then five numbers. For example, if your provincial ID is 789 then you must enter S00789. If your provincial ID is 1234 then you must enter S01234.

CADA: Please enter the number on your CADA membership card in the Provincial ID area.

SDAA: Please enter the number on your SDAA membership card in the Provincial ID area.

MDAA: Please enter your registration number WITHOUT the '070'. For example, if your number is 07030123 then you must enter 30123.

ODAA: Please enter the number on your ODAA membership card in the Provincial ID area. (Please note: If you are experiencing problems logging on and your number begins with '0' or '00', you may need to leave these off to register.)

NBDAA: You must include 'NB' in front of the number on your NBDAA membership card. For example, if your membership card number is 456, then you must enter NB456.

NSDAA: Please enter your 6 digit registration number from the Provincial Dental Board of Nova Scotia. (Please note: If you are experiencing problems logging on, please leave this field blank.)

PEIDAA: Please contact the CDAA at 1-800-345-5137 to request your Provincial ID number.

NLDAA: Please enter your 9 digit registration number from the Newfoundland & Labrador Dental Board.
