CDAA Responsibility
CDAA is responsible for protecting member personal information in its possession, including
personal information that has been received from a third party in the course of commercial activities
for processing or other purposes to which the member has consented.
Use and Disclosure of Information
We collect, use and disclose personal information to communicate effectively with members and
provide members with association information and services.
The following are some specific uses:
To open an Individual Member account
To verify Member identity
To keep track of Member activity, including use of products and services
To distribute the CDAA Journal and other communications
To provide CDAA web site access to Members
To provide information and access to CDAA Affinity programs and services
To provide CDAA Member insurance program information
To distribute receipts
To allocate appropriate educational credits
To compile statistics
To conduct market research
Personal information may be collected, used or disclosed for any purpose set out above. We will
not use or disclose your information for any other purpose without obtaining additional consent.
THE CDAA DOES NOT ALLOW THE DISTRIBUTION OF MEMBER
INFORMATION TO THIRD PARTY VENDORS. THE CDAA ONLY DISTRIBUTES MEMBER INFORMATION TO THIRD PARTIES WHO
ARE ENGAGED AS DISTRIBUTORS OF CDAA COMMUNICATIONS MATERIAL AND ARE CONTRACTUALLY OBLIGATED TO THE CDAA
TO RETAIN FULL PRIVACY OF INFORMATION.

Members have access to this information at any time by logging on to the CDAA website at http://www.cdaa.ca.
Members have the right to know which third parties have received their information from CDAA.
How do we gather your personal information?
We gather information from the following sources:
CDAA Corporate Member Provincial Dental Assisting Associations
Individual Non-Affiliated Member applications
From member interactions with CDAA
From members directly, via applications for CDAA programs and services.
From members directly in applying for CDAA affinity programs. All information
gathered in this manner is done with the consent of the member as a condition of service
and is subject to the specific privacy stipulations of the various CDAA Affinity program providers.
When a member provides personal information to the CDAA they do so with the understanding that their
personal information will be used or disclosed for the purposes previously mentioned.
Member Consent
Members consent to the use of their personal information for the purposes previously mentioned. Further
specific member consent is required before the CDAA may collect, use or disclose their personal information for
any other purpose, except in special circumstances where required by law.
Members may withdraw their consent at any time upon reasonable notice, subject to legal or contractual restrictions. Please
note that withdrawing consent may affect the CDAA's ability to provide programs and services. To withdraw consent, Members must
contact the Member Services Coordinator by regular mail (not by fax or email).
Forms of Consent
We accept any of the following as consent for the CDAA's use and future collection, use and disclosure of Member
personal information for the previously identified purposes:
Member's signature on forms
Member's express written consent as obtained through an application process
Member's consent as provided by a CDAA Corporate Member or Provincial Dental Assisting Association
Protection of Information
Whether in electronic or paper-based format, CDAA maintains a strict security system to safeguard member
personal information from unauthorized access, disclosure or misuse. When no longer needed, we take great care
to ensure the complete destruction and disposal of personal information.
In the event that CDAA transfers personal information to a third party we contractually require such
persons or organisations providing services on our behalf to protect our members' confidentiality in a manner
consistent with our own internal measures and as required by law.
The CDAA makes every reasonable effort to ensure that our Member information is accurate and up to date. Members
can access and update their personal information via the CDAA web site or by contacting the Member Services Coordinator at the address below.

The privacy of personal information is important to our members and to the CDAA. We will collect, use
and disclose personal information responsibly and only when necessary in providing services to our members. The
CDAA makes every reasonable effort to be open and transparent as to what personal information we collect, how it is handled, and
to whom it is disclosed. Our privacy policy complies with the Federal Personal Information Protection and Electronic Documents Act (PIPEDA)
and similar provincial legislation. In order to access CDAA services you must sign the appropriate form authorizing the CDAA
to collect, use, hold and disclose your personal information.
Automated Data Collection
The operating system for the CDAA website (http://www.cdaa.ca) may automatically record some general information about your visit, such as:

CDAA will be pleased to inform you of the existence, use and disclosure of personally identifying information
and provide you with access to that personal information, within 30 days of a receipt of a written request directed by
fax or mail to CDAA, Member Services Coordinator. To request a change or delete information that has previously been
furnished by you to us, make that request in writing - by fax or regular mail (not by e-mail) - to the CDAA Member Services
Coordinator with the details of your request.
